Hair thinning and excessive shedding can be frustrating, particularly when you do not know what is causing it or which treatment is appropriate.

One of the most established treatments used in hair-loss management is minoxidil. But while minoxidil is widely known, many people do not understand how it actually works, which types of hair loss it may help, how long results take or what happens when treatment is stopped.

In trichology, minoxidil can be an important part of a personalised hair-growth plan, but it is not a one-size-fits-all treatment.

Understanding the type and cause of hair loss is the first step.

What Is Minoxidil?

Minoxidil is a medication used to promote hair growth and slow certain types of hair loss.

It is available primarily as a topical solution or foam, with 2% and 5% preparations being the most commonly encountered strengths. Low-dose oral minoxidil is also increasingly used by hair-loss specialists, although oral use for hair growth is generally off-label.

Minoxidil was originally developed as a medication for high blood pressure. Its ability to stimulate hair growth was subsequently recognised, leading to its use as a topical treatment for hair loss.

Today, it remains one of the best-known treatments for androgenetic alopecia, while also having a potential supporting role in several other forms of hair loss.

How Does Minoxidil Work?

Minoxidil has a fascinating mechanism of action.

Topical minoxidil is essentially a prodrug, meaning it needs to be converted into an active form within the body. An enzyme called sulfotransferase converts minoxidil into its active metabolite, minoxidil sulfate.

This conversion takes place within the scalp and hair follicle. Individual differences in sulfotransferase activity may partly explain why some people respond particularly well to minoxidil while others experience a more limited response.

Once activated, minoxidil can influence the hair follicle and the hair-growth cycle in several ways.

1. It promotes vasodilation

Minoxidil is a vasodilator, meaning it can widen blood vessels.

Its effects on the follicle are more complex than simply "increasing blood flow", but vasodilation and changes within the follicular environment are considered part of its activity. This may support the environment required for active hair production.

2. It helps prolong the anagen phase

Every hair follicle follows a natural growth cycle.

The main phases are:

  • Anagen: the active growth phase
  • Catagen: the transitional phase
  • Telogen: the resting phase
  • Exogen: the shedding phase

Minoxidil can help prolong the anagen phase, allowing susceptible follicles to remain in active growth for longer. This is particularly relevant when follicles have become progressively miniaturised.

3. It can support miniaturised follicles

In androgenetic alopecia, genetically susceptible follicles gradually become smaller. This process is called follicular miniaturisation. Instead of producing a thick terminal hair, a miniaturised follicle may produce a progressively finer, shorter and less pigmented hair. Minoxidil can stimulate viable miniaturised follicles, potentially encouraging them to produce thicker and longer hairs.

This is one reason early assessment and intervention can be important. If a follicle is still viable and producing a miniaturised hair, there may be greater potential to improve its output than if the follicle has been permanently destroyed.

Who Can Minoxidil Help?

The most important point to understand is that minoxidil does not treat every type of hair loss in the same way.

Its role depends on the underlying diagnosis.

Minoxidil for Androgenetic Alopecia

Androgenetic alopecia, commonly known as male pattern hair loss or female pattern hair loss, is one of the main conditions for which minoxidil is used.

The condition involves progressive miniaturisation of genetically susceptible hair follicles.

Minoxidil can help support these follicles, potentially increasing hair diameter, prolonging growth and slowing visible progression.

For many people with androgenetic alopecia, treatment needs to be continued long term to maintain the benefit.

Telogen effluvium is a form of diffuse hair shedding that occurs when a greater number of follicles prematurely enter the resting phase.

It can occur following triggers such as:

  • Significant illness
  • Surgery
  • Major psychological stress
  • Rapid weight loss
  • Nutritional deficiencies
  • Hormonal changes
  • Certain medications
  • Other physiological stressors

With telogen effluvium, identifying and addressing the underlying trigger is extremely important. Minoxidil may sometimes be used as a supportive treatment, particularly when shedding is prolonged or when telogen effluvium occurs alongside another form of hair loss. It should not, however, be viewed as a substitute for investigating the underlying cause.

Minoxidil for Alopecia Areata

Alopecia areata is an autoimmune condition that can cause sudden, patchy hair loss. Because the underlying problem involves an immune attack on the hair follicle, minoxidil does not treat the autoimmune cause itself. However, it may sometimes be used as an adjunctive treatment to support regrowth alongside treatments aimed at controlling the underlying disease.

Anyone experiencing sudden, sharply defined patches of hair loss should have the condition properly assessed rather than assuming it is ordinary hair thinning.

Minoxidil for Traction Alopecia

Traction alopecia occurs when repeated or prolonged tension damages the hair follicles.

It can be associated with:

  • Tight braids
  • Extensions
  • Weaves
  • Tight ponytails
  • Certain hairstyles
  • Repeated mechanical tension

The most important treatment is to remove or significantly reduce the source of traction.

If the follicles remain viable, early intervention may provide an opportunity for recovery. If prolonged traction has resulted in permanent scarring and destruction of the follicle, however, minoxidil cannot recreate a follicle that is no longer present.

For early traction alopecia, a gentle hair-care routine is particularly important. Oil-based hair and scalp products may be useful for conditioning, reducing friction and supporting the overall hair-care routine, although they should not be considered a proven substitute for evidence-based medical treatment.

Minoxidil Strengths: 2%, 5% and Higher Concentrations

Topical minoxidil is commonly available in 2% and 5% formulations.

2% Minoxidil

The 2% formulation has traditionally been used in various patient groups and may be considered where a lower concentration is appropriate.

The ideal strength depends on the individual's diagnosis, tolerance and treatment plan.

It is important not to assume that the same product directions apply everywhere because licensing, approved indications and dosing instructions vary between countries.

What About 6% Minoxidil and Above?

Higher-strength preparations, including 6% and above, are sometimes used in specialist or compounded formulations.

However, higher concentration does not automatically mean better results.

Preparations above standard commercially approved strengths may be prescription-only, compounded or off-label, depending on the country and formulation. They should therefore be used under appropriate professional supervision.

Minoxidil Foam vs Liquid: Which Is Better?

There is no universal winner between foam and liquid. The best formulation is often the one that a person can tolerate and use consistently.

Liquid Minoxidil

Liquid minoxidil is usually applied directly to the scalp using a dropper or similar applicator.

Potential advantages

  • Easy to measure
  • Allows precise scalp application
  • Widely available
  • Often cost-effective

Potential disadvantages

  • Can leave the scalp or hair feeling wet or greasy
  • May take longer to dry
  • Some formulations contain propylene glycol, which can irritate sensitive scalps

Foam Minoxidil

Foam formulations tend to dry more quickly and may feel lighter on the scalp.

Potential advantages

  • Fast drying

  • Can be easier to incorporate into a daily routine

Potential disadvantages

  • Can be more difficult to apply precisely to areas with dense hair
  • May be more expensive
  • Can still cause irritation in sensitive individuals

Ultimately, consistency matters more than choosing the formulation that looks best on paper.

A treatment that is easy to use every day is often more practical than one that is repeatedly skipped because of its texture or application process.

What Is Low-Dose Oral Minoxidil?

Low-dose oral minoxidil, often abbreviated to LDOM, has become increasingly popular within hair-loss medicine and specialist trichology.

Unlike topical minoxidil, oral minoxidil is absorbed throughout the body. It was originally developed as an oral antihypertensive medication. Its use for hair loss is generally off-label.

Specialists may consider low-dose oral minoxidil for selected patients with conditions such as androgenetic alopecia and other forms of diffuse hair loss. One potential advantage is convenience. A person who struggles with applying a topical treatment every day may find an oral treatment easier to maintain.

However, oral minoxidil is not simply a stronger version of topical minoxidil. Because it is systemic, it can produce cardiovascular and other systemic effects.

Potential side effects include:

  • Increased facial or body hair
  • Fluid retention
  • Ankle swelling
  • Dizziness
  • Low blood pressure
  • Increased heart rate
  • Headaches

Rare but potentially serious cardiovascular complications can occur.

For this reason, low-dose oral minoxidil should be prescribed and monitored by an appropriately qualified healthcare professional.

How Should Minoxidil Be Used?

Correct application is important.

For topical minoxidil:

  1. Apply the recommended amount directly to the scalp, rather than simply coating the hair.
  2. Follow the frequency stated on the product label or recommended by your clinician.
  3. Wash your hands after application.
  4. Allow the product to dry before applying other scalp products.
  5. Avoid applying it to a severely irritated, infected or damaged scalp unless advised otherwise by a healthcare professional.
  6. Be consistent with treatment.
  7. Avoid assuming that applying more product will make the hair grow faster.

More is not necessarily better, using excessive amounts can increase the risk of unwanted side effects without necessarily improving results.

How Long Does Minoxidil Take to Work?

One of the biggest misconceptions about minoxidil is that it should produce rapid results. Hair growth takes time.Minoxidil works through the hair-growth cycle, so patience and consistency are essential.

The first few weeks

You may not notice any visible improvement.

Some people experience an increase in shedding when beginning treatment. This can occur as follicles transition through the hair cycle. An increase in shedding can understandably be alarming, but it does not necessarily mean the treatment is failing. Persistent or severe shedding should be assessed rather than automatically attributed to minoxidil.

Around 3 months

Early changes may begin to appear in some people. However, three months is often too early to make a final judgement.

Around 4–6 months

This is a more useful period for assessing whether there has been a meaningful response.

Changes may include:

  • Reduced visible thinning
  • Increased hair density
  • Improved hair diameter
  • New fine hairs in areas of previous thinning

Around 6–12 months

The response may become more apparent. Some people experience substantial improvement, while others experience stabilization or a more modest response.The response varies from person to person.

What Happens If You Stop Minoxidil?

This is an important consideration before starting treatment.

For conditions such as androgenetic alopecia, minoxidil is generally a long-term maintenance treatment. If minoxidil is helping maintain or regrow hairs, stopping treatment means that the ongoing stimulation is removed.

Over time, hairs that were being maintained by treatment may gradually thin or shed, and the hair may move back towards its untreated trajectory. This is sometimes described as "rebound hair loss", but it is more accurate to think of it as loss of the treatment benefit rather than minoxidil causing permanent additional hair loss.

Can Hair Growth Oils Be Used Instead of Minoxidil?

This is a common question, particularly among people with traction alopecia or early thinning where hairs are still present.

Hair oils are very useful in a hair-care routine.

Depending on the ingredients, they may:

  • Stimulate hair growth
  • Condition the hair
  • Reduce dryness
  • Improve manageability
  • Reduce friction
  • Support the hair shaft
  • Help maintain a healthier-feeling scalp environment

However, most cosmetic hair oils do not have the same clinical evidence for stimulating hair growth as minoxidil.

For traction alopecia, for example, reducing mechanical tension is fundamental. Importantly, if minoxidil is producing a beneficial effect, stopping it can result in gradual loss of treatment-dependent hairs. This is not talked about enough.

What happens when you stop?

When minoxidil is withdrawn, its supportive effect gradually disappears. The follicles that were being helped by the drug can return toward the hair-growth behavior they would otherwise have had.

So you can think of it approximately like this:

Underlying hair-loss tendency → minoxidil counteracts some of it → hair becomes thicker/more numerous → minoxidil stopped → the drug's effect fades → susceptible hairs gradually miniaturize/she d → coverage declines.

This is why the loss is generally gradual rather than instantaneous. For example, current U.S. labeling for 5% topical minoxidil states that if treatment is stopped, newly regrown hair is generally lost within about 3–4 months

And importantly, this isn't merely theoretical. In a small clinical study of men with pattern hair loss, researchers found that minoxidil approximately doubled the mean number of non-vellus target hairs during treatment, but most of those recruited hairs were lost after discontinuation. Some participants even ended up below their original hair count. 

Why someone can feel like they got “worse than before”

This is probably the part that isn't explained enough.

Imagine:

Before minoxidil:
You have 800 visible hairs in a particular area.

After successful treatment:
You have 1,100 visible hairs because minoxidil has thickened some existing hairs and supported additional hairs.

You stop:
The 300 additional/treatment-supported hairs don't necessarily remain simply because they have existed for a while. As the drug's effect disappears, many can be lost.

Meanwhile, your underlying pattern hair loss may have continued progressing during those years.

So you might eventually have: fewer hairs than you had when you started minoxidil.

  • Minoxidil can treat the hair-loss process in conditions such as pattern hair loss. It can stimulate growth and reduce further loss, but it does not necessarily identify or correct an underlying trigger such as iron deficiency or thyroid disease. Benefits generally require continued use.
  • Hair oils may improve the condition and appearance of the hair and scalp, but they generally aren't treatments for the underlying cause of significant hair loss.
  • Ferritin/iron, thyroid function, vitamin D, and hormones can be relevant, but not everyone with hair loss needs every one of these tests. Testing should be guided by the pattern of hair loss, symptoms, medical history, diet, medications, menstrual history, and other factors.
  • Hormone testing can be particularly relevant when there are signs of androgen excess, such as irregular periods, acne, or excess facial/body hair.
  • Most importantly, hair loss has many possible causes, including hereditary/pattern hair loss, thyroid disease, nutritional deficiencies, hormonal changes, inflammatory/scarring conditions, medications, illness, and significant weight loss. Getting the correct diagnosis matters.

What are the side effects of Topical Minoxidil

Most people tolerate topical minoxidil reasonably well, but side effects can occur.

Potential side effects include:

  • Scalp itching
  • Redness
  • Dryness
  • Flaking
  • Irritation
  • Contact dermatitis
  • Unwanted facial hair
  • Unwanted hair growth in areas where the product spreads
  • Temporary increased shedding when treatment begins

Some people find foam formulations more comfortable than liquid formulations, particularly if they react to ingredients such as propylene glycol found in some liquid products.If significant irritation develops, the product should not simply be continued indefinitely without assessment.

Seek medical advice if you experience concerning symptoms such as chest pain, fainting, significant swelling, rapid or irregular heartbeat or severe dizziness.

Why Diagnosis Matters Before Starting Minoxidil

Hair loss is a symptom, not a diagnosis. Different conditions can look remarkably similar at first.

Hair thinning or shedding can be associated with:

  • Androgenetic alopecia
  • Telogen effluvium
  • Alopecia areata
  • Traction alopecia
  • Nutritional deficiencies
  • Hormonal changes
  • Thyroid disorders
  • Medication effects
  • Inflammatory scalp conditions

This is why a proper trichological assessment can be so valuable.

The question should not simply be:

"Which hair-growth product should I use?"

The better question is:

"Why am I losing my hair, and are the follicles still capable of producing healthy hair?"

Once the underlying cause is understood, a treatment plan can be designed around the individual.

The earlier a viable follicle is identified and appropriately supported, the greater the opportunity may be to preserve and improve the hair it produces.
